Join us for a 20-minute coffee break on a new resource for state and local health departments that includes evidence-based practices for CVD prevention, the next web-based knowledge translation and evaluation mini-presentation.
To facilitate the translation of research to practice, CDC developed an easy-to-navigate "Best Practices Guide for Cardiovascular Disease Prevention Programs." The Best Practices Guide highlights eight effective strategies, backed by research evidence, that fall within two of CDC's four key domains for chronic disease prevention: Health Care System Interventions (domain 3) and Community Programs that are linked to Clinical Services (domain 4). The Best Practices Guide was created as a resource for state and local health departments look to implement evidence-based practices for CVD prevention. For each best practice strategy, it provides a high-level overview of the research evidence surrounding the strategy's ability to reduce blood pressure and/or cholesterol, reach priority populations and/or reduce health disparities, and cost effectiveness. It also provides links to implementation resources and other considerations for implementation and provides a "story from the field" describing a setting in which the strategy has been successfully implemented. The strategies highlighted in the guide include: implementing team-based care, elevating pharmacy involvement in care, engaging community health workers, activating patient self-management, implementing clinical decision support systems, and reducing patients' out of pocket costs for medications. This presentation covers the process used to develop the best practices guide and highlights the strategies that are included in the guide as best practices for CVD prevention.
