Diabetes is the 8th leading cause of premature death among Idahoans. Prevention and management of diabetes is possible through evidence-based and community based prevention and self-management programs. The purpose of this webinar is to provide healthcare providers the tools and resources needed to prevent and control diabetes. We will highlight three self-management and prevention programs available in Idaho: the National Diabetes Prevention Program (DPP), the Stanford Diabetes Self-Management Program (DSMP), and the Diabetes Self-Management Education Program (DSME).
Providers will learn the process to identify and refer patients with prediabetes to local DPP and learn how to help patients control their diabetes and minimize their risks for complications through referral and enrollment into local DSMP and DSME programs.
