During this webinar, Prevention Institute will ground participants in underlying community determinants of health, and highlight historical and current policies, practices, and procedures that have produced inequities. Building on Prevention Institute’s core tools and frameworks, participants will explore strategies for building clinical and community linkages that multiple sectors, including health care, can use to address community health factors and improve health outcomes. Participants will be able to articulate to staff and partners the connectedness and critical role of a multi-sector approach for overcoming health inequity and the systemic changes needed to produce equitable health outcomes.

Learning Objectives
Participants will be able to:
- Understand how structural drivers impact health equity and community determinants of health.
- Discuss approaches for communicating with staff and partners about health equity and social determinants of health (SDOH).
- Explore approaches to addressing community-clinical linkages to improve health equity.|
