Community Partners for Diabetes Health
Mission: Our vision is to reduce the impact of diabetes among Pierce County residents.
About Us:
Community Partners for Diabetes Health in Pierce County promotes
partnerships among local organizations within Pierce County who work to
improve the lives of people with diabetes and those who are at risk of
developing the disease, especially type 2 diabetes.
Core members include health care systesms, local and state health departments, industry and insurance representatives, pharmacys and educational instutitutions.
Our goals:
• Use the Statewide Diabetes Plan as a roadmap toward reducing the
impact of among Washington residents.
• Increase public awareness of diabetes, interventions at multiple
levels to prevent type 2 diabetes (Goals 2 in the State Plan).
• Increasing public awareness of diabetes, interventions at multiple
levels to increase self-management of diabetes to reduce
diabetes-related disease (Goal 3 in the State Plan).
