Family Health Center Overview

For over 30 years, the Family Health Center has offered primary medical care, family planning, nutrition education, and dental services for uninsured and under-served families living in Cowlitz, Wahkiakum, and Pacific Counties. We are a non-profit community health center operating clinics in Longview, Woodland, Cathlamet, Ocean Park, and opening soon in Kelso. We pride ourselves on our ability to provide exceptional care for our patients, and our sincere connection to the communities that we serve.

The Family Health Center works closely with other health care providers and specialty clinics in order to connect our patients with available resources. Our behavioral heath services continue to grow and improve, allowing patients to make better use of the health care system. We provide patient education and medical advice, helping our patients to avoid costly emergency department visits.

At the Family Health Center, we believe in community and the idea that everyone should pitch in to help each other. We hold a long tradition of partnering with community organizations to provide services and support. The Family Health Center and the Highlands Neighborhood Association regularly co-sponsor free health education sessions and flu shot clinics. We have worked with local school districts to provide back-to-school immunizations for area children. Partnerships with Kaiser Permanente have resulted in free eye exams for our uninsured patients, and support for our diabetic care management efforts. And, the Family Health Center dental clinic of Longview is a regular participant of the Cowlitz County Veterans Stand Down event, offering free oral health screenings and follow up treatment for veterans.

Care Coordination

Care Coordination is a concept that refers to a set of evidence-based, integrated clinical care activities that are tailored to the individual patient, and that ensure each patient has his or her own coordinated plan of care and services.

The care plan, which is developed collaboratively by the patient and care team, is designed and implemented to optimize the patient’s health status and quality of life.

The Family Health Center employes RN Care Coordinators and Community Health Workers to work with patients to better manage their conditions. Chronic diseases such as diabetes, hypertension, and depression oftentimes require extra attention and established care teams provide help when needed.
