Free Health Test Cards Now Available for Providers!

Starting on May 29, primary care practices will be able to order free copies of a health test card and other supporting products jointly produced and distributed by the American Cancer Society, the American Diabetes Association, and the American Heart Association (shipping and handling fees apply).  

Although the number of cards delivered will depend upon the size of the practice, the average size order includes 400 cards.  In addition to the cards, each initial order comes with a "starter kit" that includes an attractive pop-up stand for displaying the cards, a set of small tear-away pads with an individualized check list of health tests that can be used like a prescription pad in the exam room, and a wall-mount poster to hang in the office/exam room.  Pictures of the various products are attached.  

The value of the materials included in an average size kit order is estimated at roughly $400. Supplies are limited and only medical practices are eligible to order free copies.
