How to become a member

Membership is open to people and organizations based in or who have a significant presence in Washington State and who work to promote diabetes prevention and/or control.

To become a member click on the "Get Connected" link below:

        Get Connected!

1. This will take you to the "Join Form." 
2. Select all of options that apply by clicking in the box next to the description:

►If you only want to be added to email list and receive e-newletters.

►If you want to add content to the Connection.

►If you want to add your organization as a partner to the Connection.

3. Fill out the join form completely, noting that the items with a red box are required.
4. If you don't have a title or credentials, just type "none".
5. Submit.

You will receive an immediate confirmation that your application has been sucessfully submitted, followed by an email confirmation that you have been added to the email list.
